Care Coordinator / Medical Assistant (Remote, Part-Time, Utah)

EI Health is an early-stage healthcare company. We partner with physician practices to staff Medicare chronic care management programs for patients living with chronic pain and other chronic conditions. Our remote care coordinators do the monthly outreach, coordination, and documentation those programs require, under each practice’s written protocols and the supervision of its physician.
We are hiring our first Care Coordinator. You will work directly with the founder, help build the workflows, and grow with the company.
About the role
You manage a panel of Medicare patients by phone and video. Each patient gets a structured monthly check-in following the practice’s protocol including standardized questionnaires, medication list review, care plan goals, and barriers to care. Between check-ins you coordinate refills, referrals, records, and appointments, and document every minute to Medicare care management standards. Starting in January you will also help our partner practices collect required quality-measure questionnaires from their patients before visits and follow up on incomplete ones.
This role starts at 10 to 15 hours per week and grows to full-time as the patient panel expands. Calling windows are Mountain time business hours;
other work is flexible.Target start: October 2026.
Responsibilities
- Conduct monthly care management check-ins with assigned patients by phone or video, following practice protocols
- Handle between-visit patient requests (refills, referrals, appointments, records) per protocol;
escalate anything requiring clinical judgment - Document all patient interactions and log time accurately to CCM and advanced primary care management requirements
- Draft care plan updates from templates for physician review and signature
- Enroll patients by phone: consent, program orientation, verification of eligibility
- Administer standardized health questionnaires by phone or app link and record results
- Chase incomplete pre-visit questionnaires for quality-measure programs and log completeness
- Escalate concerns to the supervising physician according to written protocols
Required
- 2+ years patient-facing experience as a Medical Assistant
- Utah resident
- Strong phone and video communication, especially with older adults
- Accurate, consistent documentation habits
- Experience with an EHR (athenahealth, eClinicalWorks, Epic, or similar)
- Private workspace suitable for confidential patient calls;
reliable high-speed internet - Able to pass a background check and federal healthcare program exclusion screening
- Own computer (Windows 10/11 or macOS) with current updates, disk encryption enabled, and a headset
Preferred
- MA certification (CMA, RMA, or CCMA)
- Experience with chronic care management or care coordination
- Pain management or primary care background
- Bilingual English/Spanish
Compensation and details
- $18.00 to $21.00 per hour based on certification and experience;
paid biweekly for all hours worked - W2 employment
- Fully remote;
optional in-person daysin the Salt Lake City area - HIPAA and protocol training provided before any patient contact
